Key Job Responsibilities Leadership To lead on agreed services on design, delivery, management, and performance, constantly improving delivery and efficiency Support and deputise for the Hub Manager and Social Prescribing/Care Coordinator lead when asked/required To contribute to the delivery of ambitious key performance targets Support the Chief Operating Officer and Managing Partner & CEO on other services ensuring continuous improvement Developing strong relationships and engagement with a range of stakeholders across the PCNs, Community Providers, Member GP Practices and Locality; Developing plans for projects identified by the PCNs and support the Clinical Directors with the implementation of the projects; Service Delivery Support the Hub manager, deputise as needed, to ensure all services operate smoothly, timely, efficiently, and effectively. To support managers on agreed services creating, supporting the development, and reviewing of plans to improve services and create new services. Ensure that administrative systems and processes are in place to best support staff members to deliver services to an excellent standard. Develop policies/SOPs to support services and communicate the policies with the relevant stakeholders. Develop and communicate process changes in services to both clinical and administrative staff. Manage and monitor changes to administrative systems and communicate where these changes may affect services. Have a full working knowledge of the clinical system i.e., Systmone. Providing support to the Erewash GP Practices ensuring they can easily refer to services. Identify any operational problems occurring in services, create solutions and resolve the issues. Staffing issues and concerns to be escalated to the appropriate managers who are the line manager of the operational workforce. Demand and Capacity Carry out and monitor demand and capacity of services. Analysing demand trends over different periods of the year and amending services to meet that demand in the most cost-effective way. Communicating trend patterns with internal and external stakeholders. Communication Develop communication strategies to promote key services to both internal and external stakeholders. Communicate any changes to existing services to the relevant stakeholders. Training and Development Ensure staff have received the appropriate induction prior to working in operational services. Ensure mandatory training is completed by all staff. Sharing learning from incidents and complaints with clinical and non-clinical staff. Ensure, where appropriate that staff have time to access professional and clinical supervision. Escalate concerns with competency to the Ops Manager who has line management responsibility for the operational workforce.
