The Chief, Imaging Health Care Group (HCG) Service is responsible for the overall management, planning, organization, direction, and control of all programs within the Imaging HCG. The Chief, Imaging HCG is responsible for fiscal oversight, patient satisfaction, patient care delivery, research activities, and appropriate interface with all other services of the VA Long Beach Healthcare System. The incumbent is engaged in facility and HCG-level strategic, operational, and tactical planning.

Rumford Community Home has a standard Residential Care Unit (36 beds) and a locked Dementia Residential Care Unit (17 Beds). The Residential Care Director will be responsible for overseeing both of these units.
Essential Duties:
• Ensures every employee has a signed job description and ensures a documented clinical orientation.
• Defines duties and establishes the general pattern of delegated authority and responsibility according to the scope of that position.
• Sees that physical facilities, equipment and supplies are present to render adequate resident care.
• Establishes standards of coverage for 24 hours, 7 days a week in the residential care department and approves necessary changes.
• Recommends needed policies to the administrator and Director of Nursing for implementation. Assures all current policies are adhered to.
• Aids in the establishments of personnel policies which will assure reasonable hours of work, adequate re-numeration, security of tenure and acceptable working conditions.
• Directs and coordinates the functions of subordinates.
• Directs efficient interdepartmental working relationships
• Studies the functions and activities of other departments and interprets these to the employees of the department.
• Supervises and assists with the maintenance and satisfactory working conditions within the residential care department.
• Establishes and carries out effective communications within the nursing department.
• Sets up the budgetary request based on past requirements and future needs, and administers the allowed budget for the residential care department.
• Advises Administrator of any equipment or supplies needed. Also informs maintenance of equipment that needs to be repaired.
• Communicates with external customers as needed including venders, suppliers and emergency personnel.
• Plans, directs and participates in programs of in-service education. Keeps recorded record of state regulated training for all staff of the residential care unit.
• Selects, appoints, promotes and discharges the members of the nursing service.
• Establishes the standards of maintaining effective disciplines.
• Promotes effective working relationships with administrator, medical staff, personnel, volunteers and agencies affiliated with the facility.
• Arranges and conducts residential care staff meetings.
• Is responsible for or may delegate the ISP (Individual Service Plans) team meetings and/or preparation.
• Responsible for oversight of resident needs for outside medical providers and coordination for continued resident care.
• Responsible for or designates responsibility for the RCA process to ensure assessments reflects the condition of all residents.
• Operates within the approved personnel budget.
• Gives guidance to personnel as needed, evaluates performance, discusses progress, and need for improvement and prepares evaluation of performance as directed.
• Makes rounds on all residents as indicated. Reports any clinical needs to the Director of Nursing, or the medical director.
• Responsible for assuring accurate recording of medication and narcotics to procedure/regulation.
• Reviews periodic checks of attendance records.
• Keeps administration and subordinates informed.
• Maintains effective functioning of the organization.
• Guides staff members individually on professional matters as requested.
• Attends meetings and department head meetings as scheduled.
• Promotes good facility/community relations.
• Uses and is knowledgeable regarding information technology systems necessary to carry out job function.
• Assist in the development and participates in new employee orientation program.
• Actively participates in performance improvement, Quality Assurance, Continuous Quality Improvement (QA/CQI), and Resident Safety meetings

Education and Experience:
• High school diploma

The Chief of Community Care is the Director of the Community Care program. The position is comprised of complex managerial, clinical care coordination and administrative components, associated with critical issues affecting patient care and VA Care / Community Care and overall standards of Veteran care coordination that influence the organizational mission, health care and policy.
The Chief of Community Care is responsible for the documented outcomes at the program or service level. Program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility, VISN, or V ACO. Program is defined as a substantial coordinated group of activities impacting patients and/or employees. The Nurse IV demonstrates ( a) Leading and organizing delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning; (b) Using advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes, (c) Functioning as an expert in clinical practice and/or areas related to the assigned roles and responsibilities, (d) Systematically evaluating current practice, and formulating outcomes for groups of patients and/or organizational processes within an area of expertise, ( e) Guiding, developing, and supporting staff from a leadership perspective, and (f) Using professional standards of care, scientific evidence, and practice to evaluate programs and/or service activities.
The Chief of Community Care Nurse IV will also need to oversee assigned operations and clinical activity for care coordination, transfer center, and Traveling Veterans Coordinator, which includes organizing and coordinating the clinical components for the unauthorized/authorized care of non-VA services in response to individual healthcare needs along the illness and care continuum and in multiple settings. The goals are to center services around the patient, to foster patient self-managed care, and maximize efficient and cost-effective use of health care resources. The Chief of Community Care Nurse N utilizes all avenues of resources available to ensure staffs provide safe and efficient timely coordination of quality health care and services. The role of the Chief of Community Care Nurse N is to establish, promote and monitor seamless care for Veteran patients.
